Sara was honored as Playmate of the Month for July 2006 and selected 2007 Playmate of the Year in the June 2007 issue of Playboy magazine. Currently, Sara is a senior at Oregon State University. Sara was born and raised in Portland Oregon and in 2002, she graduated from Scappoose High School in 2002.
Sara also appeared in the reality television show, The Girls Next Door, episode #2 of E! reality television series as a playmate hopeful when she had her test shoot taken at Playboy Studio West. Sara was annointed the title of Miss July 2006. Sara has since appeared in several more episodes of the reality show, hanging out with the girls and attending events at the Playboy Mansion. On the May 6, 2007 episode of Girls Next Door (Season Three), it was revealed she, along with fellow Playboy models Alison Waite and Holly Madison, worked for Hooters at the Beaverton, Oregon restaurant (Waite and Madison worked at different respective restaurants). It also showcased Sara when Hef informed her that she was to be named Playmate of the Year.
Upon the release of the June 2007 issue of Playboy magazine, Underwood was named 2007 Playmate of the Year. Sara appeared as a pirate wench in Epic Movie, along with Playmates Qiana Chase, Jillian Grace and Irina Voronina.
